This section provides a range of resources to help health professionals support their patients experiencing homelessness. It also provides information about local homeless health plans and partnership meetings.

The StreetLink website can be used by anyone in England and Wales to send an alert about someone who is sleeping rough, or preparing to sleep rough. Alerts are sent to outreach teams, who go out mostly at night to connect people sleeping rough to local support services

Severe Weather Emergency Protocol (SWEP) is an emergency humanitarian response to severe weather conditions, the primary aim of which is to preserve life. SWEP is triggered on a pan London basis when a temperature of 0°C is forecast on any one night.

The response primarily takes the form of shelter or short-term accommodation for those who would otherwise be sleeping rough. It is coordinated at a local authority level. If unsure how to proceed, submit a referral to StreetLink (see the StreetLink subsection on this page).”
