In North West London, we implement our population health management strategy through the Focus-on methodology. The population health management workstream enhances capacity and capability across the system to ensure the successful execution of this approach.

Population Health Management (PHM) skills building

Building skills, knowledge and expertise across the system to successfully implement the locally developed Focus-on methodology and reduce inequalities across all of our work within the ICS

  • Delivering the NW London PHM and Health Equity Academy to build skills across the 4 domains to address inequalities and deliver against our collective priorities:
    • Data analytics and intelligence-led approaches
    • Working in partnership with our communities
    • Leading and facilitating PHM in the workplace
    • Understanding value and embedding evidence

Understanding and responding to need

Using population health analytics to understand need in our communities and improve our system approach to allocative efficiency

  • Aligning system resources to community need to achieve more equitable outcomes with phase 1 delivery describing the “as is” position, with phase 2 integrating an agreed needs attribution model that will support sustainable and  consistent allocative efficiency as part of a “to be” decision-making process
  • Delivering the NW London shared needs assessment and embedding into the planning and decision making process

Data & intelligence-led approaches

Developing a robust intelligence-led approach to research and evaluation to support effective decision-making based on sound evidence.

  • Embedding population health data analytics tools, infrastructure and models, with a strong focus on supporting and enabling  Neighbourhood Health teams
  • Embedding research consistently and routinely into project scoping and business cases  and, alongside evaluation, ensuring a joined up and comprehensive approach to demonstrating impact, effectiveness and value
  • Delivering an evaluation toolkit to enable consistent decision-making around what we do, and do not invest resources in through well-evidenced interventions  (www.nwlevaluationtoolkit.org.uk)
  • Integrating qualitative and quantitative data into practice, reporting and decision-making, working in collaboration across departments and specialisms to support more holistic narratives

Population Health Management Implementation

Supporting implementation of priority projects to ensure PHM is embedded as a core approach. Creating evidence of effectiveness and impact to support scaling and integration

  • Demonstrating the opportunities and benefits of innovative PHM tools and approaches (including predictive and forecasting analytics and the use of Artificial Intelligence)
  • Delivering a clear and consistent NW London segmentation and risk stratification model that enable both operational and strategic planning and delivery
