The NW London CSPC Model of Care Working Group has co-designed and agreed the overarching principles which will underpin the overall design of future community-based specialist palliative services in NW London for the next five years until 2027. Beyond this we will make sure our services have sufficient flexibility to increase service provision against a projected growth in demand, as and when that arises. These are designed to work in coordination with a set of service parameters or must dos that have also been co-designed by the NW London CSPC Model of Care Working Group to support the development of the options and services moving forward.
- Ensure the needs of NW London’s palliative and end of life patients, their families and carers are at the centre of service delivery.
- Services must be sufficiently responsive to changing (and local) needs and be culturally and community sensitive.
- We need to increase access to and increase the provision of services for ‘need to reach’ populations, especially those of minority populations.
- Any service development should be inclusive and co-designed with service users and service providers, informed by extensive engagement work and be data driven.
- Build and collaborate with existing community specialist and generalist palliative care arrangements, in addition to wider health and care arrangements where appropriate.
- Ensure alignment with the NW London ICS’ four core principles, which are:
- Improve outcomes (including e.g. service experience) in population health and care;
- Be accessible and equitable, resulting in reduced inequalities and ill health;
- Represent long term value for money and ensure efficient resource utilisation;
- Support broader economic and social development activities.
