All referrals to Adult community Specialist palliative care should be received and triaged in line with the following principles:
- There is no wrong way in for a referral – any referral to specialist palliative care will be received and processed, avoiding multiple back and forth with the referrer.
- The specialist palliative care teams are the best people to determine the right services and support for the patients and their carers.
- Initial contact should be made with the patient as soon as possible following referral.
- CSPC services across NWL are joined up and work collaboratively to ensure the care coordination and experience of the patients and their families and carers.
- There should be good communication with the referrer and the patient and those close to them throughout the referral process.

Patient choice will be factored into decisions on which services the patient receives and where.
Core referral criteria for all CSPC services
These set out the core criteria that patients must meet to be referred for any community specialist palliative care service.

Aged 18 years or over |
|
|
Diagnosis of advanced, progressive, life-limiting illness - usually an illness that worsens over time and has no cure or considerable uncertainty regarding potential for recovery. All end-stage conditions, including but not limited to: cancer, motor neurone disease, Parkinson’s disease, dementia, multiple sclerosis, chronic obstructive pulmonary disease (COPD), heart failure, kidney failure, lung disease, advanced liver disease, and HIV/AIDS. |

Complex symptoms or circumstances requiring expert management and specialised palliative care input*. Needs might be physical, social and/or spiritual and could include:
These needs can no longer be met by regular care teams in the community or primary care alone i.e. unpaid carer, district nurses, GPs, hospice at home, social care package, or continuing health care. Input is required from palliative care specialists for shared care of the patient. |
