Whether you’re part of a Place or borough team, Primary Care Network, community team, local authority, or voluntary sector organisation, this hub is designed to help you stay informed and connected as we continue to build more joined-up, person-centred care across our communities.
Communications Toolkit
The Neighbourhood Health Communications Toolkit has been created to help teams across North West London talk confidently and consistently about neighbourhood health and Integrated Neighbourhood Teams.
It includes clear, accessible messaging to support conversations with both staff and residents, helping everyone understand what Integrated Neighbourhood Teams are, and why they matter.
The toolkit includes three slide decks which can be accessed below:
- Explainer deck - a general introduction to neighbourhood health
- Public-facing deck - for use in community engagement or with residents
- Local authority deck - designed for conversations with council colleagues and partners
WSIC Data Dashboard
The North West London Business Intelligence Team has developed the Neighbourhood Team WSIC Dashboard to support a population health management approach. Available resources:
Please note that the dashboard is not yet accessible to all Local Authority and Voluntary Sector colleagues. We are actively working to expand access as soon as possible.
Case Studies
We’re starting to build a library of case studies that highlight the real world impact of neighbourhood working across North West London, and we really want to continue to grow it. These examples are here to share learning, inform local delivery, and celebrate great practice.
Current case studies include:
- Hillingdon Hypertension Model - a data-led approach to improving the detection, diagnosis, and prevention of hypertension.
- Harrow Cardio-Renal-Metabolic Model - supporting people with long-term conditions through integrated, neighbourhood-based care.

Our monthly Community of Practice brings together colleagues from across NW London to share learning, explore challenges, and support each other in delivering neighbourhood-based care.
It’s open to all clinical and non-clinical staff working in neighbourhood health, whether you’re already leading local work or just looking to get involved.
