Personalised care means working with people, not doing things to or for them. It recognises individuals as experts in their own lives and focuses on what matters most to them - supporting them to live well, manage their health, and make informed choices.

Personalised care is already happening across North West London and you have a role to play. Use the tools, learn from the examples, and start small. Every conversation counts. Whether it’s asking what matters to someone, co-creating a care plan, or connecting people to local support, small changes can make a big difference.
