Information for young people, parents and schools

Children aged 12 to 15 who have not yet had their vaccination at school can be vaccinated at a GP-led local vaccination centre. Appointments can be booked via the national booking service or see our where to get a vaccine page.

Currently 12-15 year olds are eligible for two doses. The second dose, 12 weeks after the first.

1 dose of the COVID-19 vaccine gives good protection against your child getting seriously ill. But 2 doses gives stronger and longer-lasting protection.

This page contains information about the vaccine and the consent form which you will need in order for your child to be vaccinated. You'll find the consent form, a leaflet explainer and more in the resources section below.

The consent form is an important document and your child cannot be vaccinated without it. Hence, you should download the form and complete it at home once you have read through the information leaflet held on this website.

Once you have completed the consent form you should print it out at home and take it with you when you and your child attend the local vaccination centre. You may also have the form as an electronic copy if you wish i.e. a completed signed copy on a hand held device such as a phone which can be shown to the vaccination team on the day of clinic. 


Before you book, please note the following:

Healthy 12-15 year olds currently only require one dose of the vaccine and so if your child has already had the Pfizer-Biotech vaccine at school within the last few weeks then they will not yet be eligible for another and in which case please do not book an appointment.

Please review the questions on consent form, which are also listed below, and consider if any of these apply to your child, prior to booking an appointment.

N.B. Your child must be accompanied to their vaccination appointment by their parent/s or their legal guardian.

EXCLUSION CHECKLIST (also listed on the consent form)

If you answer 'yes' to any of the below, then your child is unable to be vaccinated:

  • Has your child tested positive for COVID-19 in the last 28 days (by a lateral flow test or a PCR test)?
  • Has the individual experienced major venous and/or arterial thrombosis occurring with thrombocytopenia following vaccination with any COVID-19 vaccine?
  • Has the individual had any vaccination in the last 7 days?
  • Is the individual currently unwell with fever?
  • Has the individual ever had any serious allergic reaction to any ingredients of the Covid-19 vaccines, drug or other vaccine?
  • Has the individual ever had an unexplained anaphylaxis reaction?
  • Does the individual have a history of heparin-induced thrombocytopenia and thrombosis (HITT or HIT type 2)?
  • Does the individual have a history of capillary leak syndrome?
