Dr Lyndsey Williams (Chair) provided an update on local (Brent borough) end of life care improvements and forums, including the directory of services to support patients and clinicians in understanding and accessing palliative and end of life care services and the heart failure and end of life pathway.
This was followed by a presentation on the NW London proposed new model of care for Adults (18+) community-based specialist palliative care services by Jane Wheeler (NHS NW London Local Care Programme director) and Michelle Scaife (NHS NW London programme delivery manager for last phase of life and Universal Care Plan).
Key highlights
- Need for greater clarity on how people are assessed and referred into the services within the model of care (including whether hospitals can refer people to CSPC services).
- The length of time implementation of the new model of care will take.
- How people will be able to raise formal complaints or report issues with care under the new model.
- The frequency of quality reporting from providers and how patient feedback will be collected on services.
