Community health services are an essential component of providing person-centred, co-ordinated care. It delivers accessible NHS healthcare services to adults and children in convenient places close to their homes and, where necessary, in their homes.

Teams of nurses and therapists coordinate care, working with other professions including GPs and social care. Through assessing, treating, educating and caring for local people, they aim to improve health, enable well-being and enhance the quality of their lives.
