Acton Town Medical Centre Practice.

NHS North West London has written to patients at the Acton Town Medical Centre to give their views on the options available to the ICB given the difficult decision made in October to formally end the contact held with Dr W Moustafa and Dr G Murtaza of Acton Town Medical Centre, effective of 24 September 2025. 

We would like to take the opportunity to thank Dr Moustafa and Dr Murtaza and their staff for their many years of service and commitment to their patients and the local community.

Patients were advised that a short-term Caretaker practice had been appointed to ensure patients continued to have access to GP services whilst the ICB considers long-term options for the provision of GP services for patients.

What happens when a GP Contract ends?

When a GP Contract ends, the ICB is required to decide how patients can continue to access GP services in the future.

In deciding which approach to take we need to consider all relevant information such as the size and demographic profile of the local practice population, public health needs, the availability and condition of the existing premises, the number, capacity and the quality of other local GP practices.

What is being considered for the practice?

When making a decision, we consider the following factors:

  • Premises availability: The premises at Acton Town Medical Centre is not owned or managed by the NHS so any new service provider would be required to provide premises in the local area.
  • Number and capacity of local practices: There are seven practices within a one-mile radius of the former Acton Town Medical Centre premises. All seven of these practices have open lists and are currently registering new patients.
  • Spread of the patients registered with the practice: Patients live across a wide geographical area, with many residing outside of the borough of Ealing.
  • Number of patients still registered with the practice: Many patients have already chosen to register with other local practices. This means that the number of patients still registered at the practice is now much lower that may make a replacement service not viable for another provider to take on.
  • Patient Views: Feedback from patients on their primary care needs.

Whilst a final decision has not been made, sadly, given these circumstances, it is likely in this case that the practice will not be able to reopen, and patients will need to register with another GP practice.

How Acton Town Medical Centre patients can get involved?

We want your views to help us implement the change safely and fairly, including any support you may need, what matters most when choosing a new practice, and how we should communicate with you.  Your views will need to be provided by Monday 08 December 2025

Patient Survey

Patients are able to complete a survey where you can share your views and provide feedback on your needs. 

If you are unable to use the digital survey via the above web address, please email nhsnwl.lon-nw-pcc@nhs.net and a hard copy of the survey will be emailed or sent via post to you. Alternatively, hard copies of the survey will also be made available for you to collect at and Crown Street Surgery. These can be returned to the practice.

The ICB will also host virtual meetings through Microsoft Teams on:

Next Steps

The ICB will review patient and stakeholder feedback, alongside all other relevant information. Once this is completed, we will write again to all patients advising of the outcome.

The ICB is committed to offering the best possible access to health services for our patients and we are keen to provide all the necessary support required throughout the process.

Accessing Care in the meantime

Crown Street Surgery will continue to provide GP services to Acton Town Medical Centre patients so they can  continue to access GP care until any long term changes are agreed. 

However, patients right to choose your GP practice remains and they can choose to register with another practice if you wish to do so.
